Four of the federally qualified health centers, or FQHCs, in Southern Arizona are proud to be participants in Arizona Connected Care. It’s a natural fit for FQHCs to be part of an accountable care organization because our missions are in alignment: improving the health of the people of our region; enhancing the patient experience; and reducing or controlling the costs of patient care.

We value Arizona Connected Care’s commitment to following best practices to assure the highest quality of care. We share its priority of involving patients not only in their own care, but also in the governance of the organization. We believe by focusing on the wellness of our patients and reaching out actively to those with chronic conditions we’ll be providing the best care to the people we serve throughout Southern Arizona while helping control the increasing costs of medical care.
